It happened just before 10 a.m. at a Montefiore Medical Group building on Central Park Avenue.
At least five people were hurt, including two people in the car and three employees.
Police are looking into whether a medical episode caused the elderly driver to lose control.
One of the front doors of the office dislodged and broke through the windshield, pinning the 84-year-old driver. She was taken to the hospital in critical condition and rushed into emergency surgery.
The medical office offers primary care, urgent care, pediatrics and is usually very crowded.
It took almost four hours for crews to pull the vehicle out of the building.
The building sustained considerable damage and the structure will need to be evaluated.
